I acknowledge that my team's participation in sports and recreational activities involves risk of injury. I voluntarily assume all risks, known and unknown, and release Fair Play Athletics, its staff, coaches, volunteers, and affiliates from all liability for injuries, claims, or damages that may result from participation in this event.
I hereby grant permission to Fair Play Athletics and its representatives to take photographs and/or video recordings of my team in connection with the above-identified event.
I authorize Fair Play Athletics, its assignees, and transferees to use and publish the same in print and/or electronically for purposes including but not limited to:
✅ Social media
✅ Promotional materials
✅ Website content
✅ Press or media coverage
I understand that participation is voluntary and that no compensation will be given for use of images or recordings.
